Information for Referrers

The information in this section is intended to inform referrers of the Home Care Criteria and Hospice Care Criteria for services available to patients and their families. THHS also offers Bereavement Programs to families adjusting to changes created by the death of a loved one.

To Make a Referral

To refer your patient to our Home Care, Hospice Care or Bereavement Programs, please use our Online Referral Form or call 877-827-0788. One of our intake specialists will guide you through our easy referral process. To find the location nearest you, please visit the Locate a THHS Agency Web page.
