The Trinity Home Health Services Total Joint Rehabilitation program provides specialized orthopedic therapy that allows the patient to become physically mobile more quickly and with less pain and discomfort.
Total Joint Rehabilitation program features:
- Pre-op safety phone call (after patient referral)
- First home visit performed by a physical therapist within 24 hours of hospital discharge with advance notification of surgery by physician office or hospital
- Protimes (waived test) and staple removal performed by a physical therapist with advanced training
- Increased patient satisfaction due to consistent caregiver
- Written update to physician from physical therapist on all return patient visits
- Physician-specific functional outcomes tracked
