Partners in Care Program

Trinity Home Health Services provides its Partners in Care program to patients with chronic disease or life-threatening illnesses. This program focuses on symptom management, comfort care and quality of life during the final phase of life. People who may benefit from this program are those with a significantly advanced disease such as cancer, dementia or Alzheimer's, stroke, chronic liver or kidney disease, and others.

Partners in Care program features:

• Physician-led care planning and evaluation that gives the patient and family / caregivers support to seek desired treatment
• Pain relief and comfort care
• Link with Hospice Care and Bereavement Services if desired
• Staff competency tested in knowledge and skills related to serious illness
• Protocols to guide symptom management / comfort care
• Emotional and spiritual support
• Volunteer support services
