Obtaining Hospice Care Services

The best time to learn about Hospice Care is before it's needed.  At any time during a life-threatening illness, it is appropriate to discuss hospice. We are happy to give you information or be part of that discussion.

Too often hospice referrals are not made until the patient has uncontrolled symptoms or is near death.  Although hospice can assist and provide care at this time, the patient and family can benefit much more from hospice services if referred earlier. Many symptoms such as pain are controlled so they do not become severe, and symptoms that might otherwise require a hospitalization or an emergency room visit can be managed in the patient's home. Quality of life for the patient and family can be greatly enhanced by early hospice intervention. 

A referral to hospice is appropriate when:

• The person has a terminal illness.
• Comfort care and symptom management become the primary focus.
• Curative treatment is no longer the person's choice or option.

Referral inquiries may be made by anyone, but in order for a patient to access hospice care, they will need an order from their physician. THHS will work with the patient, family and physician to get the order. Our staff is dedicated to helping patients and families understand hospice services and working collaboratively with physicians to provide optimal comfort care and symptom management.

For families considering hospice care for a loved one, a member of our Hospice Care team can visit the patient and family at home, in assisted living facility, a nursing home or anywhere that is fitting to present information and give an assessment on how hospice services might be of assistance. There is no charge or obligation for this initial informational visit.
